CMS will soon give us its proposed changes for the Medicare hospital inpatient prospective payment system (PPS) rule that will become effective October 1, 2008 for the federal fiscal year 2009. Based on the FY08 rule, CMS is likely to move forward with significant and dramatic changes, particularly with regard to quality reporting and incentives, as well as refinements to DRG weights, and the transfer provisions. The rule will also reveal the agency's thinking on capital payments (proposed to be frozen for FY08) and further changes to "inpatient only" requirements. And, there will be other revisions, to include the outlier threshold, changes to long-term care DRGs and their relative weights, and almost certainly to ambulatory surgery center procedures.
